Do I need a prescription ?
Whether you need a prescription or not depends on the “payor” and also the type of service requested. Insurance companies, including medicaid waivers typically require a physicians prescription for therapy.
However, not all services require a prescription. Contact us for clarification.
Who can write a prescription for me?
A medical doctor (MD), doctor of osteopathy (DO), physician’s assistant (PA), nurse practitioner (NP), doctor of optometry (OD), doctor of podiatric medicine (DPM), doctor of dental medicine (DMD).
A referral to occupational therapy and our services can be made by anyone, however only the above named are allowed to write a prescription for occupational therapy services.
What should the prescription say?
The prescription must have the following information: Current Date, Your Name, Date of Birth, Diagnosis, and it must state “Occupational Therapy”
