Overview
Our organization is a nonprofit dedicated to improving access to high-quality, compassionate rehabilitative and developmental care. We provide in-home services including Physical Therapy (PT), Occupational Therapy (OT), Speech Therapy (ST), and Applied Behavior Analysis (BCBA) to support individuals in building independence, communication, mobility, and daily living skills within the comfort of their home environment.
Our care model is designed to meet clients where they are—literally and developmentally—by delivering personalized, goal-driven therapy tailored to each individual’s needs and circumstances. By focusing on home-based care, we help reduce barriers to access while promoting meaningful progress in real-world settings.
As we continue to grow, we are expanding our services to include outpatient care, allowing us to extend the same integrated, client-centered approach into a clinical setting. This expansion will further increase accessibility and provide families with flexible options for consistent, coordinated care across environments.

Our Process
1. Referral & Insurance Verification
A physician referral is required to begin services for PT, OT, SLP. Once received, we verify insurance coverage and confirm eligibility. Behavior therapy does not require a physician referral in most cases.
01
2. Evaluation & Care Plan
A licensed provider completes an in-home or telehealth evaluation and develops a personalized treatment plan tailored to the client’s needs.
02
3. In-Home, Outpatient, or Telehealth Services
Therapy is delivered where it’s most effective for the client—at home, in an outpatient setting (as available), or via telehealth. Our PT, OT, ST, and BCBA providers focus on meaningful, real-world progress across all environments.
