How to be proactive in transistiong through life stages? A life management plan from childhood to adulthood for our community.

It seems to me that we need to be prudent in creating a greater long-term plan that runs parallel to all life plans for the disabled community. We make plans for education, vocation and retirement. We need to have tools that enable those who are differently abled to make similar plans and adhere to them. This planning will ensure success as it sets expectations and how to meet them. In the area of disability and caregivers, a plan ensures little to no disruption in the event that a primary caregiver is no longer able to be present for the person. It decreases the risk of last minute scrambling associated with “not knowing” what to do. Let us make the transitioning plan part of our everyday care.
